Job Summary
Conduct comprehensive in-home assessments to evaluate physical, emotional, and environmental needs, translating findings into patient-centered care plans. Provide skilled nursing interventions including wound care, infusions, catheter care, medication management, and chronic disease management tailored to individual goals. Educate and empower patients and caregivers through clear instruction and support to help them manage daily care at home. Monitor patient progress closely, identify changes in condition early, and communicate effectively with physicians and the interdisciplinary team to adjust plans and prevent hospitalizations. Coordinate care across providers to ensure continuity and complete timely, accurate documentation reflecting the full clinical picture. Requires an active RN license with at least one year of clinical experience, a current driver's license, and reliable transportation.
Required Qualifications
- Holds an active Registered Nurse (RN) license in the state of practice
- at least one (1) year of clinical nursing experience
- Is patient‐focused, organized, and motivated by delivering high-quality care
- Has a current driver's license
- Has reliable transportation
