Provide direct nursing care in accordance with established policies, procedures, and protocols. Implement and monitor patient care plans while assessing and coordinating discharge planning needs with the healthcare team. Monitor, record, and communicate patient conditions as appropriate, serving as the primary coordinator of all disciplines for well-coordinated care. Note and carry out physician and nursing orders.
