Registered Nurse MDS Coordinator to join a Skilled Nursing Facility in New York, NY. Responsibilities include intake and evaluation of patients in long-term care, up-to-date patient assessments, overseeing interdisciplinary assessments for all nursing home patients, monitoring care procedures, and entering medical codes involved in medical billing to maintain accurate records. Lead interdisciplinary teams to ensure regulatory compliance and optimize resident outcomes through comprehensive care planning and MDS coding.
Required Qualifications
- Education: Associate’s or Bachelor’s degree in Nursing (ADN/BSN)
- Active New York RN license
- MDS Assessor Certification (required)
- 1–2 years in long-term care with MDS coordination experience
