Job Summary
Assess, plan, implement, and evaluate patient care for individuals with renal conditions using The Christ Hospital Professional Practice Model. Perform holistic assessments of physical, psychosocial, spiritual, and educational needs while recognizing and reporting abnormal data to physicians. Collaborate with multidisciplinary teams to coordinate care plans, establish measurable goals, and initiate discharge planning. Provide evidence-based interventions, delegate nursing actions, and deliver individualized patient and family education. Continuously evaluate care effectiveness and document all activities accurately in the electronic medical record. Maintain competency through mandatory training, participate in staff meetings, and mentor peers as a resource for staff and nursing students.
Required Qualifications
- BSN within 5 years of the date of hire
- Current licensure to practice nursing in Ohio
- BLS certification prior to patient contact
- ACLS certification within 6 months of hire in step-down and ICU areas
- Certification in area of clinical specialty within five years of eligibility
- 100% on-time completion and attendance for annual education day
- Ability to accept and implement change
- Ability to work in a culturally diverse setting
Desired Qualifications
- MSN or DNP
- Membership in professional organizations
- Certification specific to work area of professional practice when eligible
- Participation in the professional advancement program
- Participation in chosen professional organizations and conferences
