Coordinate care for patients with post-surgical, orthopedic, cardiac, and wound care needs by performing comprehensive assessments, developing individualized care plans, and administering medications while monitoring responses. Educate patients and families on diagnoses, treatments, recovery goals, and discharge planning while collaborating with physicians, therapists, and interdisciplinary teams. Accurately document patient care and outcomes within the electronic medical record and serve as a clinical resource and advocate.
