Job Summary
Coordinate care, support patients and families, and collaborate with an interdisciplinary healthcare team to help patients regain independence and improve quality of life. Provide safe, age-appropriate nursing care in a rehab and subacute setting, perform comprehensive patient assessments, and develop individualized care plans. Administer medications and treatments while monitoring patient responses, educate patients and families on diagnoses and discharge planning, and accurately document patient care within the electronic medical record. Serve as a clinical resource and advocate for patients and families in a compassionate, patient-centered environment focused on safety and dignity.
Required Qualifications
- Active Maryland Registered Nurse (RN) License
- American Heart Association CPR Certification
- Associate degree in Nursing
- Strong critical thinking and communication skills
- Basic computer proficiency including Microsoft Office and electronic medical records
