Job Summary
Conduct comprehensive health assessments, develop individualized care plans, and oversee medication management for NDIS participants with complex health needs. Provide clinical supervision to support workers, monitor participant status, and manage clinical incidents in line with policy. Travel to SIL, SDA, and participant homes to deliver hands-on care while maintaining accurate clinical documentation and NDIS reporting. Liaise with medical teams, families, and stakeholders to support onboarding and care plan reviews. Requires current AHPRA registration, First Aid/CPR certification, and a valid driver's license.
Required Qualifications
- Nursing qualification with current AHPRA registration as a Registered Nurse
- Sound understanding of NDIS requirements and managing complex health needs
- Excellent communication skills and the ability to build strong relationships with participants, families and team members
- Current First Aid and CPR certification
- Valid driver's licence and access to a reliable vehicle
- Yellow Card, Blue Card and Police Clearance
- Flexibility to work hours that meet participant needs
