Registered Nurse / Licensed Practical Nurse – Start of Care & Home Health Visits – Volusia County, FL

Complete Start of Care assessments and ongoing skilled nursing visits for patients in homes throughout Volusia County, including Daytona Beach, DeLand, and surrounding areas. Perform comprehensive patient assessments, wound care, medication management, and disease-management education while monitoring progress and reporting changes to the clinical team. Coordinate care with physicians, therapists, and caregivers, ensuring all visits—from routine follow-ups to discharges—are completed according to physician orders and the plan of care. Deliver accurate clinical documentation and adhere to Florida state, federal, and Medicare requirements. Requires current Florida RN or LPN license, home health experience, and ability to travel independently within the service area. Flexible per diem scheduling with weekly pay and supportive team access.
