Job Summary
Provide in-home nursing and rehabilitative care to patients in MA as part of Pathways Healthcare’s home health and hospice program. Initiate preventive and rehabilitative nursing procedures; administer medications and treatments as prescribed; conduct initial assessments of patients and families; gather comprehensive medical histories; develop and revise individualized plans of care in collaboration with physicians, nurse practitioners, and interdisciplinary teams; address inquiries from patients, caregivers, and medical teams; supervise home health aides; identify discharge planning needs and proactively implement them as part of care plans; maintain compliance with healthcare regulations; and support patients and caregivers before, during, and after treatment. Requires an active nursing license in MA, a nursing degree, BLS CPR certification, reliable vehicle, and strong communication skills; opportunity includes growth and a comprehensive benefits package.
Required Qualifications
- Active Nursing license in Massachusetts
- Nursing degree from an accredited institution
- BLS CPR certification
- valid driver's license and own a reliable, insured vehicle
- preferably 2+ years of nursing experience with a geriatrics focus
- excellent bedside manners and communication skills
- working knowledge of healthcare laws and regulations
