Job Summary
Provide and direct nursing care to patients in their homes as prescribed by physicians, coordinating total plans of care with other health care professionals to ensure continuity. Conduct initial and comprehensive nursing evaluations, determine primary care focuses, and develop plans of care within state-specific guidelines while submitting accurate documentation. Perform patient assessments at designated time points and ensure patients meet home health eligibility and medical necessity guidelines. Requires current RN licensure, CPR certification, and a dependable vehicle.
