Conduct comprehensive patient assessments, create individualized care plans, and manage a caseload of hospice patients while coordinating with the interdisciplinary team. Administer medications per physician orders, educate families on symptom management, and provide emotional support. Maintain accurate clinical documentation, participate in IDT meetings, and supervise ancillary staff to ensure continuity of care. Requires RN licensure, BSN preferred, and ability to work independently with on-call rotation.
