Registered Nurse

Provide in-home hospice care as an RN Case Manager: perform comprehensive patient assessments and create individualized care plans; manage a caseload of hospice patients; administer medications and treatments per physician orders; educate patients and families on symptom management, medication, and end-of-life care; monitor and update care plans based on ongoing assessments; provide emotional and clinical support to patients and caregivers; communicate with physicians, care partners, and community resources; maintain accurate clinical documentation; participate in IDT meetings and on-call rotation; supervise ancillary staff and ensure continuity of care.
