Job Summary
Perform comprehensive patient assessments, create individualized care plans, and manage a caseload of hospice patients by coordinating care with the interdisciplinary team. Administer medications and treatments per physician orders while educating patients and families on symptom management and end-of-life care. Monitor and update care plans based on ongoing assessments, provide emotional and clinical support, communicate with physicians and community resources, maintain accurate clinical documentation, and participate in interdisciplinary team meetings and on-call rotation. Supervise ancillary staff to ensure continuity of care.
Required Qualifications
- Registered Nurse (RN) licensed in the state of employment
- Minimum 1 year of nursing experience in hospice, home health, or a related field
- Active CPR/BLS certification
- Valid driver's license, auto insurance, and reliable transportation
- Ability to work independently and with a team
- Pre-employment drug screening is required, including testing for marijuana
Desired Qualifications
- BSN or Certified Hospice and Palliative Nurse (CHPN) preferred
