Job Summary
Conduct patient assessments, coordinate care with physicians and care managers, and deliver nursing treatment in accordance with agency policies. Manage long-term care, patient case management, and injury recovery assignments while ensuring all clinical assessments and care plans meet deadline-driven agency guidelines. Train, coach, and direct clinical staff on performance duties while maintaining compliance with state and federal laws, including Medicare and Medicaid regulations. Requires current Florida RN license, CPR certification, and valid driver's license. Schedule is flexible, built around individual needs.
Required Qualifications
- Current Florida RN license
- Current CPR Certification
- Driver's License and Vehicle Insurance
- Current TB test with negative results
- Ability to pass a Level II background check
- Understanding of state and federal laws and regulations related to home health, especially Medicare and Medicaid
- Positive, approachable, and mentoring personality
- Strong interpersonal and communication skills
- Organizational abilities
- Attention to detail
- Self-starter with a 'Go the extra mile' personality
Desired Qualifications
- Experience with Medicare (OASIS), Medicaid and commercial insurance in the home health environment
- Embrace technology and have experience with industry software such as Electronic Medical Records (EMR) and Electronic Visit Verification (EVV) systems
