Registered Nurse - Isle of Wight

Conduct in-home assessments and develop personalized care plans for clients transitioning to AmeriCare Plus in-home personal care. Coordinate admissions paperwork while ensuring compliance with agency policies, and communicate with families, caregivers, and physicians to maintain continuity of care. Support clients with compassion and clinical expertise during their transition to home care. This role requires an active Virginia RN license with at least one year of nursing experience, reliable transportation, and a valid driver's license. Located throughout the Orange area of Virginia, the position offers flexible part-time or full-time scheduling.
