Job Summary
Lead care coordination and patient progression from admission through discharge as a Registered Nurse Inpatient Clinical Care Manager. Serve as central liaison between clinical teams, payers, and patients, using evidence-based guidelines to manage patient care progression with the physician and healthcare team. Responsibilities include leading daily huddles to coordinate progression and address barriers, coordinating with RN/physician/clinical affiliate and support teams to progress care toward discharge, monitoring length of stay and discharge readiness, identifying and escalating discharge barriers or readmission risks, supporting safe discharges through timely assessments and resource coordination, educating patients and families about care plans and discharge instructions, and documenting activities in the EMR. Participate in quality improvement initiatives to reduce length of stay, prevent readmissions, and improve outcomes. Pay range: $36.25-$53.70/hour; exempt status; based on experience, skills, and education. Location: St. Joseph’s Health, Syracuse, NY.
Required Qualifications
- Education: Graduate of an accredited college or university affiliated nursing program
- BSN graduate preferred according to NYS requirements
- Licensure: Current Licensure as a Registered Nurse RN in New York
- Certification: BLS/CPR; CCM/CPHM preferred
- Experience: One to two years progressive experience in the Acute Care Setting
