Job Summary
Registered Nurse III to provide nursing and case management support: assess medical readiness, develop and monitor care coordination and case management plans, coordinate referrals and follow-up care with healthcare providers, maintain medical records and case documentation, support health assessments and readiness events, collaborate with military and civilian healthcare professionals, monitor case progress and recommend improvements to care management processes, ensure compliance with policies, procedures, and privacy requirements. Preferred BSN and CCM certification with experience supporting military, federal, or government healthcare programs.
Required Qualifications
- Active, unrestricted Registered Nurse (RN) license
- Minimum of three (3) years of nursing experience
- Experience in case management, care coordination, or related healthcare services preferred
- Strong communication, organizational, and documentation skills
- Proficiency in Microsoft Office and electronic medical record systems
- May be required to obtain and maintain a security clearance and access approvals based on contract requirements
