Job Summary
Assess patient needs, prioritize according to urgency, and determine necessary disposition via telephone or web consult using evidence-based resources. Provide patient education, medical advice, and preventative behaviors while maintaining the electronic medical record within expected timeframes. Lead the transition of care outreach program by ensuring follow-up for patients following hospital discharge or emergency room visits. Supervise the triage nursing team by assigning duties, coaching, hiring, training, and taking corrective action. Develop, enforce, and maintain policies, procedures, workflows, and clinical guidelines while engaging in quality assurance and improvement activities.
Required Qualifications
- Active RN license issued by the State of Maryland Board of Nursing or any state under the Nurse License Compact
- Minimum of three (3) years of nursing experience, with acute care
- Current BLS/CPR certification
- Excellent written, verbal, telephone, and interpersonal communication skills
- Experience documenting in an Electronic Medical Record (EMR)
- Written and oral fluency in English

- Progressive leadership experience
- Specific knowledge of eClinicalWorks (eCW)
- Prior experience in a lead or supervisory capacity
- Bilingual (English/Spanish)
