Job Summary
Conduct nursing health assessments to identify and resolve client health and safety concerns. Monitor medication effectiveness, side effects, and treatment plans according to a person-centered approach. Assist in developing community supports for individuals with complex medical needs and provide periodic reviews of health care treatments. Monitor and evaluate medical charts, nursing care guidelines, and responses as outlined in the Person-Centered Plan. Interpret physician's orders and link with health care professionals, schools, guardians, and inpatient programs as needed. Requires an RN license in Michigan, an associate or bachelor's degree in Nursing, and one year of experience with the elderly or disabled preferred.
Required Qualifications
- Possess an associate or bachelor's degree from an accredited college or university with a major in Nursing
- Possess an RN license with the State of Michigan

- One year of experience working with the Elderly or Disabled preferred
