Conduct hourly rounds to evaluate patient environments and anticipate needs like pain management and re-positioning. Construct and implement individualized plans of care that integrate direct and indirect activities for patients with specific diagnoses and co-morbidities. Coordinate interdisciplinary care by developing goals, treatment plans, and discharge strategies that address psychosocial, spiritual, and diversity needs.
