Registered Nurse ICU Patient Navigator

RN ICU Patient Navigator responsible for care coordination and serving as a central point of contact for patients and families. Facilitates transitions from the ICU to step-down units or home, reduces treatment delays, and ensures continuity of care. Eliminates barriers to timely access to services (financial, transportation, social); provides psychosocial support; and collaborates with doctors, nurses, social workers, and other care team members to optimize care workflows and outcomes.
