Registered Nurse - Hospital Liaison PT

Job Summary

Educate hospital staff and interdisciplinary teams on hospice services and programs while facilitating communication with physicians. Lead the development and implementation of strategic initiatives to increase referrals, then coordinate those referrals to ensure seamless patient transitions. Provide oversight and case management for hospitalized hospice patients, monitoring care delivery and reporting directly to the Executive Director. Work 2 ten-hour days Monday through Friday with no weekend coverage. Requires an accredited nursing graduate, current RN license, and two years of general nursing experience.

Required Qualifications

  • Graduate of an accredited school of professional nursing
  • Current Registered Nurse (RN) license in good standing with the State Board of Nursing
  • Minimum of two (2) years of general nursing experience
  • Demonstrated competency in pain and symptom management
  • Practices in accordance with accepted professional nursing standards and ethical principles
  • Demonstrates understanding of the organization's philosophy and the principles of death and dying
  • Knowledgeable of Medicare Conditions of Participation for hospice programs and all applicable federal, state, and regulatory requirements
  • Maintains current CPR certification
  • Demonstrates strong administrative and organizational skills, including effective written and verbal communication and public speaking abilities
  • Ability to work independently with minimal direct on-site supervision
  • Proven ability to collaborate effectively with physicians, healthcare professionals, and interdisciplinary team members
  • Ability to travel locally and access hospitals, nursing facilities, and other healthcare settings as required
  • Valid driver's license, reliable transportation, and automobile insurance in accordance with state and organizational requirements
  • Compliance with Bluegrass Care Navigators Employee Health, infectious disease, and vaccination policies
  • Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements
  • Demonstrates strong oral and written communication skills, with the ability to convey information clearly and professionally
  • Possesses thorough knowledge of regulatory and clinical requirements related to the provision of hospice care

Desired Qualifications

  • Preference for experience in hospice, home health, medical-surgical, or critical care settings
