Job Summary
Plan, coordinate, and deliver direct patient care from admission to discharge for the Hospital at Home program. Perform patient assessments, develop individual care and education plans, and manage discharge planning. Direct and supervise nursing care within the assigned clinical setting, delegating appropriately to others. Comply with departmental policies, regulatory standards, and quality improvement initiatives while maintaining open communication with leadership and support staff. Travel from the operations center to local hospitals and patient residences. Requires current Massachusetts nursing licensure, BLS certification, and minimum five years of nursing experience.
Required Qualifications
- Current Massachusetts Nursing licensure and registration
- Current unrestricted Driver's license
- BLS certification (unless an advance certification such as ACLS, PALS, or NRP is current)
- Minimum 5 years of Nursing experience
- Thorough knowledge of the discipline of Nursing
- Unit specific certification and competencies as defined in the Unit Profile
- Bachelor of Science degree in Nursing (Preferred)
- Homecare or Telehealth experience (Preferred)
- Phlebotomy experience (Preferred)
- IV insertion experience (Preferred)
- Telemetry experience (Preferred)
