Job Summary
Coordinate and manage direct patient care from admission to discharge for the Hospital at Home program, utilizing the nursing process to assess, plan, and educate patients and families. Direct day-to-day work responsibilities within the assigned clinical setting, delegating appropriately to support staff while ensuring compliance with Joint Commission and DPH regulations. Develop individual care plans and discharge strategies, maintaining open communication with physicians, program leadership, and support personnel to optimize patient flow and satisfaction. Travel between the operations center and patient residences to deliver care in alternative environments.
Required Qualifications
- Current Massachusetts Nursing licensure and registration
- Current unrestricted Driver's license
- BLS certification (unless an advance certification such as ACLS, PALS, or NRP is current)
- Minimum 5 years of Nursing experience
- Thorough knowledge of the discipline of Nursing
- Unit specific certification and competencies as defined in the Unit Profile
- Bachelor of Science degree in Nursing (Preferred)
- Homecare or Telehealth experience (Preferred)
- Phlebotomy experience (Preferred)
- IV insertion experience (Preferred)
- Telemetry experience (Preferred)
