Registered Nurse - Hospice - PRN

Job Summary
Lead Patient-Centered Hospice Care through comprehensive assessments, development of individualized comfort-focused care plans, and skilled nursing with emphasis on pain and symptom management. Serve as Care Coordinator within an interdisciplinary team, coordinating care with physicians, social workers, chaplains, and aides. Provide education and emotional support to patients and families, guiding them through complex decisions with empathy. Maintain accurate documentation, participate in IDT care planning, and support quality and compliance standards. Field-based care in homes and community settings with high autonomy and strong team support, focusing on dignity, comfort, and quality of life.
Required Qualifications
- Active RN license (or compact eligibility)
- Graduate of an accredited nursing program
- Minimum 1 year of clinical nursing experience
- Strong clinical judgment and compassionate care approach
- Ability to manage a caseload independently
- Excellent communication skills in sensitive situations
- Basic EMR proficiency
- Current CPR certification
- Valid driver’s license and reliable transportation
