Registered Nurse - Hospice - PRN

Job Summary
Assess hospice patients using Nursing Assessment and LCD criteria to identify physical, psycho-social, and environmental needs. Develop and document care plans while administering nursing care in patient homes with strict infection control measures. Coordinate care with the Interdisciplinary Team, attending physicians, and families to ensure individualized psychosocial needs are met. Submit all required paperwork timely and participate in QAPI programs and in-service training to maintain competency. Perform on-call responsibilities for emergency calls and travel locally to various patient sites. Requires two years of clinical nursing experience, one year in hospice, a current RN license, and valid auto liability insurance.
Required Qualifications
- Two (2) years experience as a Registered Nurse in a clinical care setting
- one (1) year full time hospice
- Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
- Current State license as a Registered Nurse
- Current driver's license
- Reliable transportation
- valid auto liability insurance
