Job Summary
Assess and admit patients, develop interdisciplinary plans of care, and document all encounters and interventions accurately in the medical record. Communicate regularly with the multidisciplinary team, including primary care physicians, and educate caregivers on providing indicated care. Address physical, social, emotional, and spiritual needs while maintaining ethical conduct and confidentiality. Work independently with minimal instruction to ensure consistent care aligned with federal, state, and hospice standards. Maintain current knowledge of regulations and participate in in-services as requested.
Required Qualifications
- Current RN license in Wisconsin or Illinois, with the ability to obtain a RN license in the opposite state. Must apply within 30 days of hire.
- Valid driver's license and reliable vehicle.
- Ability to work independently with minimal instruction.
- Maintain current knowledge of state and federal hospice regulations, and the RN Scope of Practice for Wisconsin and Illinois.
- Maintain ethical conduct and keep confidential personal and medical information about patients.
- Will assist with call schedule
Desired Qualifications
