Job Summary
Deliver patient-centered hospice homecare in the patient’s home, conducting comprehensive assessments and developing/adjusting individualized care plans. Provide expert pain and symptom management, educate and support patients and families, and collaborate closely with an interdisciplinary team. Requires strong clinical judgment, excellent communication, reliable transportation, and a compassionate, community-focused approach to end-of-life care.
Required Qualifications
- Current Registered Nurse (RN) license
- Hospice or homecare/palliative care experience preferred
- Strong clinical judgment and communication skills
- Reliable transportation
