Practice independently in the community as part of an interdisciplinary care team, delivering personalized nursing and care management to patients in their homes or care facilities. Constantly evaluate evolving patient needs and respond with plan of care adjustments. Maintain a current New York State license with at least one year of medical/surgical experience; Wound Care Certification is preferred. Earn a salary between $112,209 and $138,409 annually. Required Qualifications
