Registerd Nurse Hospice Home Care Franklin provides high-quality hospice nursing care in patients' homes in the Franklin area. Responsibilities include conducting comprehensive assessments, developing and updating individualized care plans, administering medications and treatments, and providing symptom and pain management for patients with life-limiting illnesses. The role involves collaborating with physicians, social workers, hospice aides, spiritual care providers, and family caregivers to ensure coordinated and dignified care aligned with patient goals. Education for patients and families about disease progression and available support services, offering emotional support during end-of-life transitions, prioritizing visits, documenting care in electronic health records, and participating in on-call rotation as needed. Required Qualifications
