Registered Nurse - Hospice - FT

Hospice Registered Nurse leading patient-centered hospice care in field-based settings. Responsibilities include conducting comprehensive patient and family assessments, developing and managing individualized comfort-focused care plans, delivering skilled nursing care with emphasis on pain and symptom management, acting as care coordinator within an interdisciplinary team, educating and supporting patients and families through the journey, and ensuring accurate documentation while participating in care planning and IDT meetings. Requires empathy, strong communication, clinical judgment, and the ability to independently manage a caseload; experience with Homecare Homebase (HCHB) is a plus. Field-based, autonomous role with strong team support and a focus on dignity, comfort, and quality of life.
