Job Summary
Provide support to Hospice RN Case Managers by ensuring patient needs are met and high-quality care is maintained across private residences, skilled nursing facilities, and personal care homes. Complete visits for multiple case managers, assist with new admissions, and manage referrals while participating in on-call rotations to ensure continuous patient care. Utilize strong assessment skills to manage symptoms proactively and interact empathetically with patients, families, and interdisciplinary team members in emotionally challenging situations. Work autonomously to handle various patient care scenarios with professionalism, adhering to accepted ethical and legal standards of nursing practice.
Required Qualifications
- Graduate of an accredited school of registered nursing
- Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or through a participating Compact State Board of Nursing
- Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements
- Maintains current CPR certification
- Adheres to accepted professional, ethical, and legal standards of nursing practice
- Demonstrates understanding of and commitment to hospice philosophy and the principles of death and dying
- Provides compassionate, patient-centered care while serving diverse populations with respect and cultural sensitivity
Desired Qualifications
- Minimum of one (1) year of general nursing experience preferred; new graduate nurses will be considered
