Registered Nurse- Hospice (Field Case Manager)

Job Summary
Field-based Registered Nurse (Hospice) coordinating and implementing the overall plan of care for assigned patients across hospice settings to ensure quality of life. Responsibilities include providing all aspects of care for terminally ill patients, patient and family education, completing regulatory requirements for hospice nurses, performing patient assessments and documentation, symptom management, developing a comprehensive Interdisciplinary Plan of Care (IDT), orienting new staff, and participating in Quality Improvement activities. Requires driving and a Florida nursing license, with at least 1 year of Medical-Surgical nursing experience (Hospice experience and Hospice/Palliative certification preferred). Must work within an interdisciplinary team and maintain strong communication skills; capable of caring for pediatric, adolescent, adult, and geriatric patients; proficient in EMR documentation; available to work 24/7 with holiday coverage as required. Halifax Health affiliation noted.
Required Qualifications
- Graduated from an accredited nursing program
- Valid Florida license to practice as a Registered Nurse
- Minimum 1 year experience in Medical Surgical nursing or equivalent
- Hospice Experience preferred
- Certification in Hospice/Palliative preferred
- Valid driving license and good driving record
- Ability to work within the interdisciplinary team framework and demonstrate good oral and written communication skills
- Skilled in providing nursing care to pediatric, adolescent, adult and geriatric patients
- Demonstrates good interpersonal skills, maturity and emotional stability
- Meets or exceeds productivity standards
- EMR documentation competency
- Knowledge of end-of-life care and pain/symptom management
