Registered Nurse - Hospice Case Manager

Job Summary
Deliver compassionate, comprehensive health care services to hospice patients in home care settings, private residences, skilled nursing facilities, or Hospice Care Centers. Create and implement holistic plans of care based on patient goals, manage symptoms through pharmacologic and non-pharmacologic interventions, and serve as the patient advocate to coordinate care for families and caregivers. Conduct frequent monitoring and follow-up to evaluate treatment efficacy while providing emotional support to patients and their caregivers. Work autonomously within an interdisciplinary team to maintain optimal comfort, functionality, and safety for individuals navigating end-of-life care.
Required Qualifications
- Graduate of an accredited school of registered nursing
- Currently licensed as a Registered Nurse through the Kentucky Board of Nursing or through a participating Compact State Board of Nursing
- Minimum of one (1) year of general nursing experience preferred; new graduate nurses will be considered
- Demonstrates strong clinical nursing skills and the ability to maintain professionalism during emotionally challenging situations
- Provides compassionate, patient-centered care while serving diverse populations with respect and cultural sensitivity
- Adheres to accepted professional, ethical, and legal standards of nursing practice
- Demonstrates understanding of and commitment to hospice philosophy and the principles of death and dying
- Maintains current CPR certification
- Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements
