Job Summary
Provide weekend daytime support to hospice and home health patients in Northern Colorado through scheduled visits, admissions, and PRN assignments. Administer skilled nursing care in accordance with physician orders and the plan of care, conducting initial evaluations, determining immediate needs, and revising care plans as conditions change. Furnish specialized nursing procedures, initiate preventive and rehabilitative interventions, and counsel patients and families on nursing-related needs. Coordinate services with the Inter-Disciplinary Team, physicians, and caregivers while documenting progress notes, OASIS assessments, and discharge summaries. Supervise LPNs/LVNs and HHAs, assign home health aides, and participate in quality management, peer review, and staff teaching. Take on-call duty nights, weekends, and holidays as assigned.
Required Qualifications
- Graduate of an approved school of professional nursing
- Currently licensed in the state(s) in which practicing
- Maintains current CPR certification
