Registered Nurse

Conduct comprehensive patient assessments and develop individualized plans of care for patients recovering from illness, injury, surgery, or managing chronic conditions. Provide skilled nursing services including wound care, medication management, IV therapy, and disease management education while monitoring progress and adjusting care plans. Educate patients and families on treatment plans, coordinate with physicians, therapists, and interdisciplinary team members, and maintain accurate clinical documentation. Identify changes in patient condition and communicate appropriately with the healthcare team to ensure compliance with agency policies, state regulations, and Medicare guidelines.
