Job Summary
Per Diem Hospice Admit Nurse in San Luis Obispo to perform initial patient assessments for hospice needs, counsel the patient and family, provide education related to their needs, and collaborate with an interdisciplinary team to promote patient and family choice and quality of life. Requires self-directed organization with the ability to manage a caseload and flexible scheduling, with work Fridays and one weekend day (Sat or Sun) from 8 AM-5 PM and no night calls.
Required Qualifications
- Current California RN License
- CPR certified (Healthcare Provider)
- Reliable vehicle to drive for work
- Current Driver's license and clean driving record
