Job Summary
Assess patient and family for physical, psychological, social, and spiritual needs to establish, implement, and evaluate the plan of care. Initiate communication with the attending physician, medical director, and interdisciplinary group to coordinate optimal care and resource use. Document according to protocol, maintaining up-to-date records that reflect baseline status, current interventions, and ongoing changes. Provide education to patients and caregivers based on their specific knowledge needs and learning assessments. Participate in on-call or admissions assignment rotations as directed.
Required Qualifications
- Licensed as a Registered Nurse in the state(s) of practice
- Previous nursing experience in hospice, home health, general medical-surgical, or population-specific care (e.g., pediatrics, geriatrics)
- Willingness to serve on-call
- Reliable vehicle
- Valid driver's license
- Minimum required liability auto insurance
- Strong communication and organizational skills
- Working knowledge of Microsoft Office Suite and EMR systems
- Ability to work effectively as part of an interdisciplinary team
- Physical ability to lift patients over 200 pounds (with assistance), stand, walk, sit, stoop, and perform other physical movements frequently
Desired Qualifications
- Associate degree (preferred)
- CPR certification (recommended)
