Job Summary
Conduct compassionate, trauma-informed nursing care and health navigation for individuals experiencing homelessness by engaging clients in shelters, encampments, and community settings. Assess health needs, provide basic medical care, promote wellness, and connect people to healthcare and social service resources. Drive daily to transport clients to appointments while working within a multidisciplinary outreach team. This role supports Thresholds Homeless Outreach Program's mission to link people with serious mental illnesses to stable housing and primary psychiatric care.
Required Qualifications
- Passion for mental health advocacy
- Desire to provide compassionate care
- Effective communication and relationship-building skills
- Good writing skills
- Strong time management and organization
- A valid driver's license
- Current car insurance
- Daily access to your own car
- Willingness to provide transportation to clients
- Registered Nurse license
- Associate's, Bachelor's or Master's degree from an accredited school of Nursing
