Job Summary
Staff Nurse Homecare with VNA Health Group provides in-home nursing care to patients in Monmouth County, NJ. Responsibilities include assessing patients with complex health needs, developing personalized care plans, educating patients and families about conditions and care paths, and documenting changes in patient conditions to update care plans. Collaborates with VNA clinicians and external providers to ensure seamless care, and participates in a two-week clinical orientation covering procedures, EMR system, safety protocols, and compassionate care. Requires driving to patient homes and utilizing clinical equipment; emphasizes patient education, chronic condition management, and transitioning patients to better health. Ideal candidates hold an RN with a bachelor's degree (preferred) and have 1+ year of RN experience, with strong communication skills and proficiency in EMR tools. Pay range reflects New Jersey Pay Transparency Act.
Required Qualifications
- RN license in New Jersey
- Bachelor's degree in Nursing preferred
- 1+ year clinical RN experience
- valid driver’s license and access to reliable transportation

- willingness to travel to patient homes
