Assess patients with complex health needs, develop personalized care plans, and educate families on their conditions and treatment paths. Collaborate with clinicians and external providers to ensure seamless care, while documenting patient progress and updating care plans. Complete a two-week clinical orientation covering procedures, EMR systems, and safety protocols before beginning. Requires a registered nurse with a bachelor's degree and one year of clinical experience, plus a reliable vehicle and valid driver's license. Compensation ranges from $43.00 to $51.00 hourly with a $2,500 sign-on bonus.
