Job Summary
Registered Nurse Case Manager provides skilled nursing care and coordinates end-of-life hospice support for patients in home or facility settings. Responsibilities include conducting comprehensive patient assessments, developing individualized care plans, coordinating with physicians and the interdisciplinary team, educating patients and families on symptom management and disease progression, documenting assessments and care plans in the EMR (HCHB), participating in care-planning meetings, and supporting after-hours needs through an on-call rotation when applicable. Requires an active RN license and driving; hospice or home health experience is preferred; strong assessment, coordination, communication, and documentation skills; familiarity with HCHB is preferred.
Required Qualifications
- Active Registered Nurse (RN) license
- Active driver’s license
- Ability to pass a criminal background check
- Ability to pass a Motor Vehicle Record (MVR) check
- Current TB testing in accordance with agency policy
- Current CPR certification as required for the role
