Job Summary
Provide hospital-level care directly in the patient's home by completing thorough nursing assessments, creating individualized care plans, and delivering advanced skilled interventions for acute conditions like heart failure, COPD, and post-hospitalization needs. Monitor patient status using remote physiologic monitoring, coordinate comprehensive care including diagnostics and medication reconciliation, and document efficiently using Point-of-Care electronic records. Support safe transitions and assist with discharge planning through patient and caregiver teaching. Work with a supportive interdisciplinary team, starting and ending the day with virtual team rounds to ensure continuity. Manage your daily schedule with autonomy while caring for up to five patients per day.
Required Qualifications
- Graduate of an approved, accredited school of professional nursing
- Current license to practice as a Registered Nurse in the Commonwealth of Massachusetts
- Current CPR certification
- Minimum 1 year of recent acute care, home care or emergency nursing experience
- Strong clinical assessment and critical-thinking skills
- Ability to work independently and adapt to changing patient needs
- Current driver's license, a good driving record, and reliable transportation
- Commit to a minimum of 4 shifts per month (8:00 AM – 8:30 PM), with two of those falling on a weekend day
