Job Summary
Conduct home visits to assess patient environments and provide training in self-care for Peritoneal Dialysis or Hemodialysis. Evaluate patient performance, document all nursing services in the Electronic Medical Record, and communicate assessment results to the physician and team. Administer medications as ordered, obtain laboratory tests, and report critical values promptly. Coordinate home evaluations with technical services to ensure equipment requirements are met and monitor water sample results. Perform on-call nursing services nights and weekends on a rotational basis, maintaining flexible hours to accommodate patient needs.
Required Qualifications
