Job Summary
Provide in-home direct patient care according to the provider's prescribed plan of care while closely monitoring progress and recovery. Assess and document patient conditions, administer medications and IV fluids, and perform wound care and hygiene assistance. Collaborate with providers and allied health professionals to ensure exceptional patient outcomes. This role requires an Associate degree in Nursing, current RN licensure, and CPR/AED certification, with practical trach or ventilator experience preferred. Schedule includes afternoon and weekend shifts with flexible full-time, part-time, or PRN options.
Required Qualifications
- Associate degree in Nursing (AND) or Associate of Applied Science in Nursing (AAS)
- Current unrestricted license to practice as a Registered Nurse (RN) in the state associated with this position
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
