Job Summary
Registered Nurse (RN) to provide professional nursing services in clients' homes across Port St. Lucie, Fort Pierce, and Palm City. Responsibilities include planning, coordination, and assurance of quality care; developing work methods to facilitate high-quality care; delivering professional nursing services and case management; monitoring client needs and clinical outcomes; initial assessments and progress toward goals; ongoing communication with patients, families, physicians, and the care team; participating in interdisciplinary conferences; implementing and adjusting the care plan with patient representation; completing clinical documentation including OASIS, evaluations, orders, and care coordination; liaising with community health resources (DME, oxygen, infusions); interacting with patients, families, team members, and other health professionals; and engaging in ongoing education for professional development. Requires active RN license and at least 1 year of clinical experience.
