Registered Nurse-Home Health Visits

Job Summary
Make home visits to clients in designated Lehigh County territories to provide 1:1 care for adult and geriatric patients. Follow designated care plans while performing assigned duties including medication administration, wound care, and treatments. Monitor client conditions and report changes to Clinical or Client Services Managers. Execute and document doctors' orders, perform client assessments, and manage case coordination. Accurately record observations, interventions, and evaluations using a state-of-the-art touch pad tablet. Requires a current Pennsylvania RN license, one year of recent clinical experience, and ability to travel to cases as assigned.
Required Qualifications
- A current license as a Registered Nurse in Pennsylvania
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases as assigned
Desired Qualifications
- Prior home care experience
- Prior experience with electronic medical records (EMR)
