Job Summary
Conduct home health visits for adults and seniors in Delaware County, administering medication, wound care, and treatments while following designated care plans. Perform client assessments, monitor conditions, and report changes to managers. Execute and document doctors' orders using a touch pad tablet, managing case coordination and accurately recording observations and interventions. Maintain independence and time management while traveling to assigned cases.
Required Qualifications
- A current license as a Registered Nurse in Pennsylvania
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases as assigned
Desired Qualifications
- Prior home care experience
- Prior experience with electronic medical records (EMR)
