Job Summary
Conduct home health visits throughout Carbon County, administering medications, wound care, and treatments while following designated care plans. Monitor client conditions, report changes to managers, and execute doctor's orders. Perform client assessments, manage case coordination, and accurately document observations, interventions, and evaluations using a touch pad tablet. Requires a current Pennsylvania RN license with at least one year of clinical experience. Base pay ranges from $52.00 to $64.00 per point.
Required Qualifications
- A current license as a Registered Nurse in Pennsylvania
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases as assigned
Desired Qualifications
- Prior home care experience
- Prior experience with electronic medical records (EMR)
