Job Summary
Conduct home health visits to adult and geriatric clients in designated Lehigh County territories, administering medications, wound care, and treatments per designated care plans. Perform client assessments, monitor conditions, and report changes to the Clinical or Client Services Manager while accurately documenting observations, interventions, and evaluations on a touch pad tablet. Execute and follow up on doctors' orders, manage case coordination, and ensure compliance with infection prevention protocols and state mandates. Requires a current Pennsylvania RN license, one year of recent clinical experience, and ability to travel to assigned cases.
Required Qualifications
- A current license as a Registered Nurse in Pennsylvania
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases as assigned

- Prior home care experience
- Prior experience with electronic medical records (EMR)
